Declaration of Intention



Invalid for all purposes seven years after the date hereof





State of Colorado; In the District Court

of Adams County



I Antonio Agazio, aged 36 years, Occupation: Farmer, do declare on oath that my personal description is: Color: white, complexion dark, height 5 feet 7 inches, weight 140 pounds, color of hair Black, color of eyes Dark Brown, other visible distinctive marks none. I was born in San Giovanni Italy on the 7 day of January, anno Domini 1886. I now reside at Henderson RD. I emigrated to the United States of America from Naples on the vessel (can not remember). My last foreign residence was San Giovanni. I am married and the name of my wife is Mary. She was born at Pedace Italy and now resides at Henderson RD. It is my bona fide intention to renounce forever all allegiance and fidelity to any foreign prince, potentate, state, or (unreadable), and particularly to Victor Emanuele III King of Italy, of whom I am now a subject. I arrived at the port of Quebec and by train to Michigan. (unreadable) from Michigan to (unreadable), on or about the 1st day of November 1910 to Colorado. I am not an anarchist; I am not a polygamist nor a believer in the practice of polygamy; and it is my intention in good faith to become a citizen of the Untited States of America and to permanently reside therein; SO HELP ME GOD.
